A Permit To Work form allows accurate recording of checks and safety procedure methods used for carrying out potentially hazardous work by certain people on construction sites, along with signed declarations by those personnel ultimately responsible.

They describe the precautionary checks put in place (via a Risk Assessment) enabling work to be carried out efficiently and safely, clearly explaining how long it will take, how it will be done (via a Method Statement) and what work will be done.

Various features of Permit to Work Forms include: complete customer contact details; location of the work; date; work description; permit number (sequential); work activity covered by the Permit; isolation of electric, Gas or Alarms required; any known hazards; Control Measures taken to reduce risks; Authorised Site Personnel signatures.

Other general Permit variants include features such as: Hazard identification which could be Physical, Chemical, Electrical, or Site Hazards; Tools and Equipment required; PPE Personal Protective Equipment required; any supplemental Permits with the type and number; General Permit to Work conditions on-site.

Permit to Work Confined Spaces: which features: Permit details with Date Required with start and finish dates; with start and finish times; Location of Confined Space; Purpose of Work required; Method Statement number; Risk Statement Number; COSHH Assessment Number. Safety Equipment and PPE identified including Escape Sets, B.A. Sets, Harness, Safety Barriers, Gas Monitors, Lifeline, Safety Torch, Hard Hats, Man riding Winch, Forced Ventilation, Resuscitation Unit, and First Aid Equipment. The Confined Spaces variant also requires Gas Monitor readings prior to entry; Name of person taking first reading plus all persons associated with the entry; Name of Company or Contractor plus its senior contact person; Appropriate method of Communication; Emergency Procedures; Authorised Person's Signature; Close-out and Signatures that area has been made safe, workers withdrawn and keys issued.

Permit to Enter a Duct: which features: Permit details with Date Required with start and finish dates; with start and finish times; Access required; Description of access; CSP Numbers; Risk Assessment prepared and its reference; Method Statement prepared and its reference; Names of people entering the area; Name of Contractor with contact name and telephone number; specific customer requirements; statement of other permits required including Hot Works, Electrical Permit; Confined Spaces; Working at Heights, and Mechanical Isolation; Acknowledgement of responsibility by signatures of authorised personnel; Close-out and Signatures that area has been made safe, workers withdrawn and keys issued.

Permit to Enter Hot Works: which features: Permit Number; Task Description, Location and Number of Individuals carrying out the Hot Works task; Tools and Equipment required; PPE Personal Protective Equipment required; any supplemental Permits with the type and number; General Permit to Hot Works conditions Risk Assessment on-site outlining Sprinkler systems or fixed fire systems operating; whether fire extinguishers or hosepipes are available; way to sound Fire alarm; suitable protection for walls, floors and gulleys; isolation and purge of vessels and lines from flammable liquid; clearance of, protection from combustible material; wetting down of area or use of a Fire Blanket; standby Firefighter in attendance; continual Gas monitoring; any specialist safety equipment used; Atmospheric Testing required; declaration that authorised persons have been trained to carry out tasks safely; Authorised and Competent Person's Signature.

Permit to Work at Heights: which features: Permit Number; Permit details with Date Required; with start and finish times; Location of work; Description of Work required; Checks on Scaffolding usage; checks on a Safe System of Work with a Site Specific Assessment and a Method Statement; checks on Machinery Usage with certificate availability, Daily Plant Checklist used, surrounding area being cordoned off; checks on condition, suitability and security of Access Equipment; safety signage displayed; Tower Scaffolding erection procedure checks including hand-rails, toe boards, brakes and access trap door installation; Tube Scaffolding erection including surrounding area being cordoned off, boards being laid flat with no overlaps, sufficient hand-rails, toe boards and brick guards in place; access unobstructed check; sufficient number of barriers and safety signage used, plus completion of Hand-Over Certificate; Scaffolding Company's name and contact telephone number; Contract Number; Names of personnel on project; declaration that authorised persons have been trained to carry out tasks safely; Authorised and Competent Person's Signature.

Permit to Work Electrical: which features: Permit Number; Task Description, Location and Number of Individuals carrying out the Electrical task; Tools and Equipment required; any supplemental Permits with the type and number; Hazards and Precautions including checks on qualifications of personnel, advise of Isolation of electricity; whether electrical supply has been switched off; if Flammable substances have been removed; if Fire extinguishers are required; if a Fire blanket is available; have all Combustibles been removed; if a Voltage Detection instrument is required; is an Earth required; are Circuit Breakers required; of there are Caution or Danger signs displayed; if insulation mats and screens are required; do personnel need to wear protective gloves; if the Isolator is locked off; has conduction test been implemented; will sparks need to be contained; are water sprinklers in service against fire risk; are there heat resistant screens in place; is PPE Personal Protection Equipment required. Also further checks on operatives carrying out Electrical work including an understanding of the permit conditions, fire safety precautions; authorised person's can stop operatives work; Hazard reporting; declaration that authorised persons have been trained to carry out tasks safely; Authorised and Competent Person's Signature.
