A CVI form, also called a 'Confirmation of Verbal Instruction' procedure form, or 'Oral Confirmation Sheet' is used by construction contractors and builders to give the contract's administrator the legal power to issue site instructions to the Contractor.
CVI Form General Features include: the Company's name & address; Date; Contract Title; Job No; Site Instructions with big area for description; Drawing & Sketch attached tick boxes; Photos taken on-site tick boxes; Client Signature panel; Company Signature panel. A CVI number from sequential numbering process is optional.

We can provide a quote before-hand though if you've a complicated brief.
